Procedures for Requesting Disability Related Campus Accommodations
Students with disabilities need to complete the following process for disability-related accommodation requests to be considered:
- Register with Disability Support Services:
- Meet with Disability Support Services representative and complete necessary forms:
- Data Sheet
- Release/Exchange of Information
- Accommodations Checklist and/or Accommodations Request Form
- Other
- Provide DSS with current written appropriate disability documentation from a licensed physician, psychologist, audiologist or other recognized appropriate disability/medical professional or specialist.
These requirements have been established because non- standardized tests and incomplete or outdated assessment reports do not enable the DSS staff to accurately assess the student's accommodation needs. The goal of DSS is to ensure equality of access and opportunity for students with disabilities by providing accommodations and services that will best assist the student in meeting the requirements of his or her particular academic program.
C. Grace Period for Documentation Out-of-date and incomplete documentation is frequently enough to receive accommodations on a temporary basis (a grace period) while more adequate documentation is being pursued. Please call the Learning Disabilities Specialist at 208-885-6307 for more information.

2. Accommodation Checklist needs to be completed every semester for accommodations to continue. The student will deliver the checklist to their professor/instructors for signatures and return the signed form to DSS. Completed, signed checklists will be kept on file in DSS. Copies will be made for the student and each faculty member.
Note: Accommodations requested must be appropriate to and related to the disability/health condition described in the documentation provided. It is the student’s responsibility to provide this documentation and in an appropriate time frame. It is also the student’s responsibility to keep DSS informed in a timely manner of any changes that would affect their disability-related accommodations.
